Passport Canada Forms:
There is often confusion as to what Passport Canada forms notaries can sign. A notary cannot act as a Guarantor on a passport application unless the notary has known you personally for at least 2 years. However, we can notarize:

• A statutory declaration concerning lost, stolen, damaged, destroyed or inaccessible Canadian passports or travel documents, available at the Passport Canada website; and

• A statutory declaration in lieu of guarantor, available from Passport Canada service locations.
